The Empire Plan offers a personalized web site for subscribers at www.myuhc.com. Here you can review your claims history and print out copies of your Explanation of Benefits forms. Flex Plan The Flex Plan (a.k.a. Cafeteria Plan) offers our members significant tax savings on their excess medical and child/elder care expenses. Here you can download a claim form and find information on what expenses are eligible for reimbursement. Registration is now required to access the site. |

Annuities The OMNI Group, the company that the district has employed to administer its 403(b) plan, maintains an active web site at http://www.omni403b.com. Once there click on “Clients”/ “Client Home Pages” and then select “Long Beach City SD (NY)” from the list of organization’s home pages. Here you can download forms, find answers to frequently asked questions and review the list of participating providers. |
